Description of Services
Initial No Obligation Consultation - $75
The initial
consultation ranges from one to 1-1 1/2 hours and provides an
opportunity for families to ask questions, review the
various planning options and determine how they would like
to proceed.
Individualized Hourly Planning Assistance - $60/hour*
(billed in 15-minute increments)
Letter of Intent
A very
important and valuable “instruction book” on how to take
care of your loved one with a disability. A must to
complete your life care planning process. Electronic
template available for $95.
Transition / IEP Strategies
The transition
process for IEP’s is mandated to start at age 16.
Assistance is provided to make the sure IEP does include
the transition section and that it is age appropriate.
Additional consultation is available for families who
need IEP advocacy/meeting attendance. Transition also
includes preparation for adult life after high school
and putting a plan into place with supports and
resources lined up to insure a smooth and successful
transition.
Special Needs Trust / SSI
My personal
experience may be valuable to you in providing
information on the programs offered by the Social
Security Administration, and outside resources to set up
and fund a special needs trust.
Guardianship
Guardianship
should be considered around the age of 17˝. There are
many implications to full guardianship and other options
are available for those families who do not have the
need to obtain full guardianship.
Person Centered Planning
Dreams,
abilities, interests, strengths, weaknesses, needs,
etc., are discussed with the individual and their team.
We concentrate on self-advocacy and self-determination
at each meeting because they are the foundation of this
type of planning. Meeting every two months with the PCP
team is highly recommended. The
Full Life Ahead workbook
is a very useful tool in conjunction with the transition
planning process and to compliment the IEP.
Parent Mentoring
Available on
an hourly basis or on a flat retainer. I have many other
resources that will compliment your life care planning
efforts – DDD, independent
living options, parent and family support groups,
community resources, Voc-Rehab, working, transportation
and college issues, and IDEA laws.
Relationship Awareness
for
Individuals with Developmental Disabilities©
Deborah spent four years developing these materials
after her daughter had an traumatic experience with
a male classmate. Deborah asked her local school
district about their special ed sex ed and was told
there was no such thing. So, she set out to create
materials that could have been taught to her daughter a
few years back. Deborah conducts presentations
throughout the year and was selected by the AZ Dept
of Education in 2005 in a Call for Presentations for
their annual transition conference, held in
September 2005. She spends time improving the
materials and actively promoting curriculum-based
instruction with various school districts throughout
the state.
Deborah received a
copyright on the relationship materials from the
Library of Congress in June 2005. Her section on
relationship safety and self-awareness has been
included in the next edition of The
Full Life Ahead
workbook, due out in April 2006.
